Greater Manchester Mental Health NHS Foundation Trust (GMMH) provides inpatient and community-based mental health care for people living in Bolton, the city of Manchester, Salford and Trafford and a wide range of specialist mental health and substance misuse services across Greater Manchester, the northwest of England and beyond. We also provide Health and Justice Services in a number of custodial settings.

The post holder will work within an integrated, later life multi-disciplinary team within Greater Manchester Mental Health NHS Foundation Trust. The post holder will provide assessment for service users and carers, in line with the Care Act (2014). The post holder will act as a care co-ordinator as required in line with the Trust Care Programme Approach (CPA) guidance and policies.

The post holder will be an Approved Mental Health Professional (AMHP) or willing to undertake the relevant training, and will carry out the statutory duties of an AMHP in accordance with the relevant legislation and policies.

They will work to the HCPC Standards of Proficiency for Social Workers in England.

The post holder will report to the Team Manager for day to day management. Professional social work supervision will be provided by a Senior Social Worker.

The post holder will aim at all times to promote the independence and well-being of adults with mental health needs; assist social inclusion and valued community presence and enable the widest possible expression of chosen lifestyle.

Key Aims:

  • Promote and provide expertise and guidance to the wider multi-disciplinary team on a range of social work interventions, which acknowledge the social context, human rights and self-determination of the individual. Advise health colleagues in respect of key legislation and social policy which inform social work practice.
  • Assess and monitor the mental health and social care needs of service users and carers as part of an integrated team, sharing information in an appropriate and secure manner using both electronic and paper-based systems.
  • Work in partnership with other statutory, voluntary and community agencies working towards greater social inclusion for services users and carers.
